Early diagnosis of Charcot foot has traditionally been managed with offloading and immobilization until the inflammation phase calms down. With advances in diagnostics and surgical fixation, timely diagnosis can offer intervention before collapse. These techniques are generally minimally-invasive and offer advantages of less infection and wound complications seen with open approaches required for dislocated Charcot feet. In this obese 45yo Diabetic, surgery was performed to: (1) remove the contracture of the Achilles tendon which is the main deforming force with a Charcot foot collapse. Through a 2cm long incision, the fibers transitioning the calf muscle and Achilles tendon are released. When we cut something tight, the tissues immediately gap apart. With the ankle splinted at a 90 degree, this gap distance is set and the tissue regenerates in the defect to again allow a functioning calf muscle-Achilles unit which is no longer tight. When looking at the pre and post surgery xrays, you can see that the Achilles was initially so tight it was pulling the heel bone off the ground. In the post-op xrays, you can see the heel is relaxed and again sitting on the ground surface. (2). stabilize the medial column of the foot in effort to prevent advanced Charcot changes including subluxation and arch collapse. The xrays show an intramedullary beam that was inserted inside the medial column bones to stabilize their alignment. This serves as an internal support system, similar to rebar in concrete. Interlocking screws on both sides of the nail prevent migration.
